What we have learned about this county

• Every hospital in Boulder County sits on the eastern plains, but a large share of the county's land is mountain. Census TIGER figures put the county at 740.5 square miles of land and water combined, running west to the Continental Divide, and the western third is national forest and wilderness rather than road network. A patient in Nederland, Ward, Jamestown or Allenspark reaches an emergency department only by descending a canyon — Boulder Canyon/CO 119, Left Hand Canyon, or the South St. Vrain/CO 7 corridor out of Lyons. These are single-route corridors with no parallel alternative, so a mountain dialysis or appointment run has to be planned against a live CDOT closure check on the day, not just a map estimate.
• CO 119, the Diagonal Highway, is the artery between the county's two hospital clusters: Foothills Hospital and the Della Cava pavilion in Boulder at one end, Longs Peak and Longmont United in Longmont at the other. There is no comparable second route between them — the alternatives (US 36 through Lyons, or 287 through Lafayette and Erie) are both substantially longer. Every Boulder-to-Longmont transfer, discharge and standing dialysis run rides on that one highway, so check its status before quoting a pickup window.
• Boulder County's dialysis capacity sits in exactly three cities. A full pull of the CMS dialysis facility registry for Colorado returns only four Medicare-certified dialysis facilities physically inside the county: two in Longmont, one in Boulder and one in Lafayette. There is no dialysis clinic in Louisville, Superior, Erie, Lyons or Nederland, and no Fresenius or US Renal Care facility anywhere in the county. A Superior or Erie patient on a Monday/Wednesday/Friday chair is committed to a cross-county round trip three times a week, every week. Three of the four — DaVita Boulder, Kidney Center of Longmont and Kidney Center of Lafayette — run a late shift, so evening return legs are a standing requirement, not an exception.
• City names are a bad proxy for county here, and it changes who authorises a trip. Longmont and Erie both extend into Weld County; Superior extends into Jefferson County; Lafayette, Louisville, Erie and Superior all touch Broomfield. The trap is concrete: Boulder Community Health's Erie Medical Center at 101 Erie Parkway carries an Erie address and a BCH name, but the parcel geocodes to Weld County, not Boulder. For county-administered or brokered non-emergency medical transport the authorising county can differ from the one on the patient's mailing address — confirm the county at intake by address, not by city name.
